I had that problem once because my sling was too long and I was used to shorter one.  Depending on your style of throw, when you release it can be related to the length of the sling.

If it's not your release, it might be your pouch.  What size and shape is it?

If the pocket is short, you have to use a "close grip" to keep stones from falling out.

A "close grip" is one where the retained and released cords BOTH exit your fist at the same place; generally pinched between thumb and forefinger.

There was another thread that discussed the same problem.  Try using an open pouche, that is, a pocket that has a small split to it.  that usually would keep the stone more secure.

Get in a safe area where the stone will not hit anything important.
Then load your stone and just swing it around without releasing it.
This will give you a feel for keeping even tension on the cords. Usually it is "slack" in your cords due to bad technique.
I sling stones larger than tne pouch all the time.

Centrifical force will keep it secure.

It can only come out if it is not centered in the pouch---more weight is outside than inside the pouch or if there is slack that allows the stone to move out because centrifical force has been lost.

Hope that helps.  Then again, a split pouch presents a different story. I was assumming a solid pouch Undecided

Would that be the diamond-shaped cutout I see in the center of some slings?

Also, choosing an egg-shaped stone often keeps it from rolling out.

If you use a wide grip, you also should carefuly fit lenghs of the cords, maybe using the try and error method.

as they say practice makes perfect but I still use a draw string pouch mine is about four inches long and two inches wide put the stone in and give a tug cinching the pouch around it
...

Somewhere on the forum there is his design with two straps. I think the both ones are very ingenious.
