It should hang in the same direction as the palm of the hand. Here's a couple of images. In the first image I'm holding a bit of the cord, but it's just to get the pouch in frame. The orientation hasn't changed.

I had a sling with one side of the pouch that slowly got slightly longer than the other...it never functioned quite right once that happened.
